Just fill in the blanks, click the "subscribe" or "unsubscribe" button; then click the "submit" button. You only need to click 'submit' once.

Note: With some browsers, you may get a blank mail document when you click the "submit" button. If this happens to you, just put the information in the mail message and send it. I'll take care of the rest.
